INFORMATION TEXT FOR PATIENTS/SERVICE RECIPIENTS UNDER THE PERSONAL DATA PROTECTION LAW
DR. REFİK MUTLU attaches importance to protecting the confidentiality of your personal information. (As Dr. Refik Mutlu, pursuant to Law No. 6698 on the Protection of Personal Data ("KVKK"), as the Data Controller, we would like to inform you about the purposes of processing your personal data, the legal reasons for processing, the method of collection, the parties to whom it may be transferred, and the rights granted to you under the KVKK. Considering the data security of our patients and their relatives, and due to the importance we place on the protection of privacy and fundamental rights and freedoms, we would like to inform you about the processes related to the rights granted to you under the KVKK.
What Personal Data Do We Process?
To provide healthcare services to our patients, we collect various personal data in accordance with the principles and conditions set forth in the KVKK and other relevant legislation (Private Hospitals Regulation, Regulation on Personal Health Data, etc.). To provide healthcare services to our patients, the following data is collected by our hospital from our patients, other healthcare institutions and organizations, or patients' relatives for the purposes of medical diagnosis, examination, treatment, and care.
Your Identity Information: Your name, surname, Turkish ID number, Your driver's license, passport number, or temporary Turkish ID number, place and date of birth, marital status, gender, insurance or patient protocol number, and other identifying information that can identify you.
Your Contact Information: Your address, telephone number, email address, and other contact data; voice call recordings kept by customer representatives or patient services in accordance with call center standards; and personal data obtained when you contact us via email, letter, or other means.
Your Financial Information: Financial data such as your bank account number, IBAN number, credit card information, and billing information.
If you visit our hospital, we collect camera and photo recordings kept for security purposes. If you use parking or valet services, we provide your license plate number and related photos and camera recordings.
Your closed-circuit camera footage and recordings during your visit to our hospitals.
Your Health Information: General and specific personal health data obtained during the conduct of all medical diagnosis, examination, treatment, and care services, and provided by you. Your data includes, for example, your appointment and examination information, laboratory results, test results, check-up and prescription information, data specified in the consent form in the event of a medical intervention, survey, suggestion, satisfaction, thank-you, and complaint data, navigational information we obtain through email, website contact form, and the use of our website and mobile applications; your IP address, Wi-Fi information, browser information, and other data you share with us when you contact us through other means, such as medical documents, surveys, relevant registration and form information you submit with your consent through the same system, and private health insurance data for the financing and planning of healthcare services.
For What Purposes Do We Process Your Personal Data?
Your Personal and Special Personal Data listed above are used for:
To fulfill our legal obligations under the Health Services Fundamental Law No. 3359, the Decree Law No. 663 on the Organization and Duties of the Ministry of Health and its Affiliated Institutions, the General Hygiene Law No. 1593, the Patient Rights Regulation, the Private Hospitals Regulation, the Regulation on Personal Health Data, and other relevant regulations;
To protect public health, provide preventive medicine, medical diagnosis, treatment, and care services, and plan and manage healthcare services and their financing; To carry out all medical diagnosis, examination, treatment, and care services, and to procure medications, consumables, and specialized materials;
To provide you with information regarding your appointment if an appointment is made; To verify your identity; To bill; to plan, manage, and monitor your transactions quickly and accurately;
To reach financial agreements with contracted institutions regarding healthcare services provided to you; Responding to requests from public institutions and organizations in accordance with legislation
Measuring and increasing patient satisfaction following your receipt of healthcare services; Responding to all your questions and complaints; Contacting you for information regarding our services;
Developing, monitoring, and carrying out quality processes;
Researching and analyzing your use of healthcare services to improve the services we offer;
Participating in campaigns and providing campaign information through all means of communication by relevant departments;
FOR PURPOSES
To be processed, recorded, stored, preserved, and classified, and to provide information to prosecutors, courts, all judicial authorities, the General Directorate of Civil Registration, the Turkish Medical Association, the Pharmacists' Association, and relevant public officials upon request and in accordance with legislation regarding matters related to public safety and legal disputes that may arise; When necessary, it MAY be transferred to authorized public institutions and organizations for the purposes specified above, to institutions and organizations to which referrals are made in the event of a referral, to healthcare institutions and organizations with which we collaborate for medical diagnosis, to laboratories, to our business partners, to private insurance companies, to law firms, to financial advisors, and to all our consultants among the third parties from whom we receive consultancy.
What are the Methods of Collection of Your Personal Data and the Legal Grounds Related to These?
Your personal data mentioned above may be processed for the purposes specified in Articles 5 and 6 of Law No. 6698, provided that (i) it is expressly provided for by law, (ii) the processing of personal data belonging to the parties to a contract is necessary, provided that it is directly related to the establishment or execution of a contract, (iii) it is necessary to protect the life or physical integrity of the person who is unable to give his consent due to a de facto impossibility or whose consent is not legally valid, or of another person, (iv) it is necessary for the data controller to fulfill its legal obligations, (v) it is necessary for the legitimate interests of the data controller, provided that it does not harm the fundamental rights and freedoms of the data subject, (vi) for the protection of public health, preventive medicine, medical diagnosis, treatment and care services, and the planning and management of healthcare services and their financing, within the scope of the purposes specified in this Information Text; It may be processed, recorded, stored, preserved, classified, and transferred by automatic means (e.g., camera recordings) or non-automatic means (e.g., forms filled out by you) provided that it is legally compliant and, where necessary, up-to-date.

The relevant patient/service recipient who has approved this Disclosure Statement in accordance with the law and relevant legislation;
They have been informed that they have the right to learn whether their personal data is being processed,
to request information regarding the processing of their personal data,
to access and request their personal health data,
to learn the purpose of processing their personal data and whether it is being used in accordance with its intended purpose,
to know the third parties to whom their personal data has been transferred, both domestically and internationally,
to request correction of their personal data if it is incomplete or inaccurate,
to request deletion or destruction (anonymization) of their personal data,
